Today, I am going to show you how to fill out an infant check-up questionnaire.
If you are going to a leisurely hospital, it is better to go to the hospital, write it yourself, and ask questions about what you do not know.
But usually hospitals are not idle. And my baby cries whenever he goes to the hospital.
So, I’m going to show you how to make it at home.
There are three ways.
Write on the healthy person site (including the healthy person app)
The first is to write it on a website called Healthy People.
If you are using a computer, I think this method will be easier.

Access the site through the link above.
Do you see the infant questionnaire/developmental screening in the orange square box? Click.
It tells you to log in. log in. When you log in, your baby will appear on the screen and you will see a button that says “Create a questionnaire”. (It appears that capture is not possible due to security policy.)
You must first apply for the service before clicking Create Questionnaire.

After applying for the service and clicking “Create Questionnaire”, simply fill in your personal information and fill out the questionnaire below.
The app is the same. Since the menu is the same, you can download and fill out the Healthy In app from the Google App Store. 좋은뉴스
Download the questionnaire (print it out and fill it out)

This is a questionnaire for all ages from 4 months to 71 months.

Using the tick tock app
The third is to use the tick tock app. Since I have a baby, I often go to the hospital from vaccinations to the hospital, so I downloaded an app called Tick Tok. Reservations and currently open hospitals were listed well.
However, you can also fill out an infant check-up questionnaire at Tick Tok.
First, select an infant checkup item, then find a hospital and press Reservation.
Select the treatment item and date and time to complete the hospital reservation.
When you click Reservation, the page to fill out the infant checkup questionnaire appears as shown below.
There are Infant and Toddler Screenings and Developmental Screenings.
Just fill it out and click OK.
I left this method and went to the hospital to fill it out. Sobbing..
I didn’t know then. I just went out of my mind.
